Thank you for using the ATM Request system to request refills on your current prescriptions and request other patient services. Please note that you must be a current patient to use this system to request medications unless you are requesting an OTC product or supplement out of category 6. Requests for medications that you are not prescribed will be ignored and subject to both a 10% cancellation fee and a $50 non compliance fee. All prices include requisite prescription processing fees. Standard processing, compounding, and shipment may take up to 14 business days, please be sure to allow enough time for your refill. Remember compounded medications are made individually, for you, when needed.
Please note that if you are not a patient, are not in compliance, or wish to cancel a request that has already been billed to your credit card, a 10% cancellation fee will apply to cover our merchant processing fees and office expenses. You MUST be a patient of our practice to use this online system to request ANY medications INCLUDING B-12 injectable and Hair Loss Formula!
If you are a NEW patient, you may use this system ONLY to pay for your initial consultation until you have been approved to request medications and other patient services. Payments for New Patient Consultations, Procedures, and Advice Only Consultations are non-refundable. Once you submit your medical history information and it is reviewed by the Doctor, services have been rendered regardless of whether you follow through with your visit to our office if required. Advice Only Consultations do not require a trip to our office as you are purchasing advice only, you will not be considered an established patient of our practice. We are pleased to offer flexible rescheduling if necessary with 24 hours advance notice.
